1. 1At home

Tell us what sleep and daytime life look like

Before the appointment, you will complete a focused questionnaire and sleep diary. This gives the clinician a pattern to explore, not a score to treat in isolation.

    You provide
Sleep timing, symptoms, health history and current medicines
    You receive
    Clear preparation for the consultation
  2. 2Video

    Build a clinical explanation together

The clinician will ask about nights and days, breathing, movement, mood, work, substances, medicines and anything that changes the picture.

    You provide
Your priorities and the impact the problem is having
    You receive
An explanation of likely causes and the next decision
  3. 3If needed

    Test the question that matters

If obstructive sleep apnoea is suspected, a home testing device may be posted to you. You will sleep in your own bed and return the device after recording.

    You provide
One usable night of recording and any symptom context
    You receive
Clinician interpretation, not only an automated number
  4. 4Treatment

Follow a plan matched to the diagnosis

That might mean CBT-I, a short and reviewed course of medicine, practical changes, GP liaison, or referral to a service better placed to help.

    You provide
Feedback on what is changing and what is difficult
    You receive
A written plan with endpoints, review and clear boundaries
